Welcome to the MCC Medical Clinic!

In order to serve you well we are asking all new patients to register online. Once you fill out the registration form our scheduling staff will contact you to make your first appointment and obtain any other information that may be needed.

Resources & Information

Please make sure that you bring the following document on your first visit:

  • Photo ID such as driver's license or state-issued ID

  • Insurance card(s)

  • Payment via MasterCard, Visa, American Express, or cash. Personal checks may not be accepted. 

  • Complete list of medications you are currently taking, including any over-the-counter medications, vitamins or supplements 

  • Complete list of other physicians you see, their contact information, and why you see them

  • Results from your last mammogram, colonoscopy, lab work, X-rays, cardiac studies or any other tests performed prior to this visit.

Please plan on spending 1 to 2 hours on your first visit, and note that the physician runs on a 15-minute cancellation policy. If you arrive 15 or more minutes late for your scheduled appointment, you may be asked to reschedule it. In case you are unable to keep your appointment, we require 24 hours advance notice.  You may be charged a no show fee if you fail to follow this policy. Please make sure that our practice accepts your insurance plan. If you do not have any health insurance, you will be treated as “uninsured” patient. Our office will collect all co-pays, co-insurance, and deductibles at the time of your visit. If you are uninsured you may qualify for subsidized health care under the Montgomery County Cares program. Please provide all information asked for in the registration forms so that we may serve you better.

Patient Registration Form

At our clinic, the patient registration form is an important part of the patient intake process. This form allows us to collect information about the patient, including their personal information, medical history, and insurance information. Completing the confidential form prior to your appointment helps to speed up the check-in process when you arrive.
