We are pleased to welcome you to our practice and look forward to our first meeting with you. Please browse through our website to find out valuable information about our practice. You should also know that beside internal medicine, we offer several specialty care services. Please familiarize yourself with your right as a patient and Clinic's expectations from its patients. We have posted clinic policies on referrals, prescription fill ups, appointment scheduling, late arrival, and cancellation. If you are unable to find the information you are looking for, please go to Contact page and send us an email. We will do our best to find your answer as expeditiously as possible.
Our new patients can get a head start on their first visit by downloading, printing and filling out our new patient registration forms. Just use the link below to download our registration forms as PDF* documents. Please be advised that it is very important to our practice to keep up-to-date records. We may ask you to periodically update your Registration Form. If you have any changes to your demographic information, please update your form and let one of our staff members know.
If you are unsure of how to answer any portion of the forms, we will be more than happy to answer any questions for you. Our front office personnel will be ready to provide any assistance you may need. We encourage you to bring your medications with you on your initial visi. This will eliminate any confusion pertaining to dosages. Please make sure that you bring the following document on your first visit:

Payment via MasterCard, Visa, American Express, or cash. Personal checks may not be accepted.

Complete list of medications you are currently taking, including any over-the-counter medications, vitamins or supplements

Complete list of other physicians you see, their contact information, and why you see them

Results from your last mammogram, colonoscopy, lab work, X-rays, cardiac studies or any other tests performed prior to this visit.
If you must bring underage child or children with you, please make sure that you keep them under your supervision during your entire visit. Children are not allowed to accompany patients in the examination room and they cannot be left alone in the waiting area.
Please plan on spending 1 to 2 hours on your first visit, and note that the physician runs on a 15 minute cancellation policy. If you arrive 15 or more minutes late for your scheduled appointment, you may be asked to reschedule it. In case you are unable to keep your appointment, we require 24 hours advance notice. You may be charged a no show fee if you fail to follow this policy. Please make sure that our practice accepts your insurance plan. If you do not have any health insurance, you will be treated as “uninsured” patient. Our office will collect all co-pays, co-insurance and deductibles at the time of your visit.
